How soon after surgery should I start physical therapy?

Sooner than most people expect. Knee and hip replacement patients typically begin PT the day of or day after surgery, still in hospital. ACL reconstruction patients start within the first week. Rotator cuff and spinal fusion patients usually begin formal outpatient PT around 4 to 6 weeks post-op, once the repaired tissue has established enough integrity to tolerate movement. Starting too late is one of the most common reasons recoveries stall.

Does physical therapy hurt after surgery?

There will be discomfort — that is normal and expected. Sharp pain that worsens or lingers for days after a session is a signal something needs adjusting. At Intecore we work in what we call the productive discomfort zone — pushing hard enough to drive progress, backing off when the body signals it is too much. The goal is never to make you hurt. It is to make you stronger.

How long does post-surgical physical therapy last?

Most outpatient PT programs run 6 to 12 weeks, attending two to three times per week. More complex procedures like spinal fusion or ACL reconstruction may require a longer program. Your PT will reassess progress at each phase and adjust accordingly — the program should match where you actually are, not where a generic schedule says you should be.

Do I need a referral to start physical therapy after surgery?

In California, you can see a physical therapist directly without a physician referral for up to 12 visits or 45 days. What if I am still in pain months after surgery?

Persistent pain after surgery is sometimes caused by scar tissue restriction, muscle weakness, compensatory movement patterns, or sensitisation of the nervous system — all of which physical therapy directly addresses. If your recovery has plateaued or pain has lingered beyond what your surgical team expected, a fresh PT assessment is a sensible next step.

How do I choose the right physical therapist after surgery?
